Job Summary

This position is responsible for the coordination and delivery of addiction treatment services designed to meet the recovery and wellness needs of clients within the Project Free Baby & Me and ALAD addiction treatment program depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumer's family members.

Under the supervision of the Chief Nurse Executive & FMBM RN Care Manager, the Substance Use Screener will provide drug testing for clients referred to the Baby & Me Program. The screener will follow all of the established protocols for the program as detailed in program description. The staff member will ensure all documentation procedures are done in accordance with agency policies and procedures, ensuring appropriate assessment tools are utilized, recorded, and that the appropriate treatment referral is made according to the ASAM and NJSAMS level of care recommendations.

To meet the goals of the Merck for Mothers Safer Childbirth City initiatives, clinician will be responsible for screening patients to identifying social needs and potential disparities on site at the Capital Health OB/GYN - Trenton. Will work closely with RN Care Manager to connect patients and their identified family members to resources and services to achieve improved maternal health outcomes.

Essential Functions

Clinical Services:

  • Conduct initial and ongoing assessments involving the collection, analysis, and recording of significant bio-psychosocial information.
  • Licensed Clinical Social Worker (LCSW) may can assess, diagnose, and treat mental health conditions
  • Facilitate and provides information/sessions for individuals and groups to reduce resistance to addiction treatment and enhance engagement.
  • Collects specimens for substance abuse analysis
  • Treatment plans for clients on medication prescribed by program staff will be done in consultation with the prescriber as necessary.
  • Supports clients' strengths and resources, affirming reality testing, contact with experience, judgment and connection with support systems including family, friends and community resources
  • Consults with other service providers and documents their participation in the recovery process
  • Assures appropriate treatment linkages are made
  • Maintains established program productivity requirements

Documentation

  • Enters reports and assessments into information technology system as indicated by the Program Director
  • Maintains accurate and up-to-date case records in accordance with agency standards, accreditation and funding source requirements
  • Assures that nil HIPAA standards are followed with regards to client information

Productivity

  • The clinical screener will be required to meet benchmarked productivity levels. All schedules, reports, statistical reports/logs, and Clinical reports are to be submitted within proscribed timelines.
  • Documentation should be completed in Millenium within 24 hours of service delivery
  • Provides daily, weekly, and/or bi-weekly individual staff clinical supervision as necessary related to staff experience level and or clinical need.
  • In conjunction w5th the program supervisor provides feedback to treatment team regarding referrals as clinically necessary

Other Duties

  • Participate in the Quality Improvement process and specific quality initiatives for the program.
  • Utilize supervision and training as opportunities for enhancing professional competence.
  • Performs all duties in a professional and ethical manner consistent with agency, licenses, COA, and professional standard.
  • Other related duties as assigned.
  • Fill out all required paperwork and logs associated with the client’s substance use test.
  • Greet clients and treat them with respect as they present for screening.
  • Prepare UDS screening SA samples for sample pick-up and shipment in accordance with established protocols and procedures.
