Job Summary
This position is responsible for the coordination and delivery of addiction treatment services designed to meet the recovery and wellness needs of clients within the Project Free Baby & Me and ALAD addiction treatment program depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumer's family members.
Under the supervision of the Chief Nurse Executive & FMBM RN Care Manager, the Substance Use Screener will provide drug testing for clients referred to the Baby & Me Program. The screener will follow all of the established protocols for the program as detailed in program description. The staff member will ensure all documentation procedures are done in accordance with agency policies and procedures, ensuring appropriate assessment tools are utilized, recorded, and that the appropriate treatment referral is made according to the ASAM and NJSAMS level of care recommendations.
To meet the goals of the Merck for Mothers Safer Childbirth City initiatives, clinician will be responsible for screening patients to identifying social needs and potential disparities on site at the Capital Health OB/GYN - Trenton. Will work closely with RN Care Manager to connect patients and their identified family members to resources and services to achieve improved maternal health outcomes.
