This position is eligible for telework for applicants who reside in Minnesota or in a bordering state, with supervisory approval and satisfactory performance. If you live in a state bordering Minnesota, you must live within 50 miles or less from the primary work location to be eligible for telework.

The Home and Community Based Services (HCBS) Coordinator serves to support the Money Follows the Person (MFP) federal demonstration rebalancing grant. This position coordinates and supports the participants, providers, and lead agencies through the enrollment, eligibility, and delivery of home and community-based services through the participant’s first year of community living.

In Minnesota, this project is named Moving Home Minnesota (MHM) and Moving Home Minnesota - Tribal Initiative (MHM-TI), respectively. MFP grant goals are to support the transition of Medicaid participants from institutions to community-based living , and to rebalance Minnesota’s long-term services and supports to achieve a sustainable, quality service system. This position is a critical component to the success of the participant’s transition to the community, as well as the success of the MHM/MHM-TI projects.

The MFP/MHM project is currently funded through 2027, with the possibility of continued funding or to become a permanent program.

Minimum Qualifications

1 year professional experience in Long-Term Care services and HCBS and supports OR 3 years advanced technical or paraprofessional experience. Experience must include the following:

  • Experience with Minnesota Health Care Programs (MHCP), policies and procedures
  • Experience handling eligibility criteria for Medicaid/ Medical Assistance, including LTC and HCBS
  • General knowledge of MMIS, MAXIS, EDMS, CRM or similar systems used to document eligibility and enrollment

HSP Rep 1

2 years professional experience Long-Term Care services and HCBS and supports. Experience must include the following:

  • Experience with Minnesota Health Care Programs (MHCP), policies and procedures
  • Experience handling eligibility criteria for Medicaid/ Medical Assistance, including LTC and HCBS
  • General knowledge of MMIS, MAXIS, EDMS, CRM or similar systems used to document eligibility and enrollment

Preferred Qualifications

  • Skills in communications, human relations, and leadership to build collaboration and teamwork among various division and agency staff who have wide-ranging and sometimes competing agendas
  • Skills and ability to clearly and succinctly communicate verbally and in writing
  • Understand Medicare and Medicaid eligibility for purposes of enrollment, managed care program enrollment indicators, differences among managed care products for special needs populations, services included, service areas and populations served
  • Variety of experiences working effectively with others from different backgrounds and/or cultures
