Healthcare Recruiters seeks a full-time Masters level Social Worker (LCSW preferred but will accept ASW/ACSW) for our client’s 501 (c)(3) nonprofit organization that provides housing and mental health support to individuals with intellectual and developmental disabilities.

• Down Syndrome
• Autism
• Traumatic Brain Injury
• Intellectual disabilities & dual diagnoses

Key Responsibilities

• Manage full case management cycle:
• Assessment, planning, authorization, advocacy, monitoring, documentation
• Develop individualized case plans and provide ongoing support
• Conduct initial and follow-up assessments and community visits
• Link individuals to outside services: Medical care, outpatient treatment, financial assistance, etc
• Maintain accurate, compliant documentation
• Advocate for client rights, services, and benefits
• Collaborate with internal teams and external providers
• Respond to crises and provide de-escalation support
• Keep supervisor informed on caseload status and unmet needs
• Some evening/weekend coverage depending on caseload

Work Environment & Schedule

• Monday–Friday with flexibility for evenings/weekends
• Hybrid or in-office options based on program needs
• Travel between community sites, meetings, and events
• Must be comfortable in office and community settings
• Valid driver’s license & reliable transportation required

Qualifications

• Master’s degree in: Social Work, Psychology, Counseling, or Sociology
• LCSW preferred (ASW / ACSW accepted)
• 3+ years working with DD/ID and/or dually diagnosed individuals
• Experience in: Nonprofit or social services
• Case management & mental/behavioral health
• Regional Centers, FHA, or similar agencies
• Crisis intervention & risk assessment skills
• Strong communication and teamwork abilities
• Experience with: Individual/group therapy
• Multidisciplinary teams
• Therapeutic arts (art, music, dance) a plus
